Shortly after takeoff from Panama City-Marcos A. Gelabert Airport Runway 01, while climbing to a height of about 300 feet, the twin engine airplane entered an uncontrolled descent and crashed in a grassy area located about 700 metres north of the airport, bursting into flames. One occupant was killed and a second was seriously injured.

En route, while cruising along the border between Panama and Colombia, the aircraft collided with trees and crashed on the slope of a wooded mountain, bursting into flames. All four passengers were killed and both pilots were seriously injured.

Shortly after takeoff from runway 36, while climbing to a height of 50 feet, the crew raised the landing gear when the aircraft stalled. It landed back on the runway about 380 metres from the runway end. It slid on all this distance, overran then turned to the left and came to rest in a grassy area about 60 metres past the runway end. All 13 occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The stall that occurred during the initial climb was the result of the copilot's confusion in interpreting the information transmitted by the captain which led him to initiate the rotation before reaching the speed of 95 knots (V1). The crew could not obtain sufficient power from both engines because they had not activated the methanol water system in view of the existing meteorological conditions, the wet runway and the takeoff weight, which remains a contributing factor.

After touchdown on a wet runway following heavy rain falls, the aircraft was unable to stop within the remaining distance. It overran, lost its undercarriage and came to rest in bushes. While descending at an altitude of 7,500 feet in relative good weather conditions, the crew informed ATC about their position when contact was lost. The wreckage was found five days later at the altitude of 762 metres on the slope of a mountain (850 metres high) located near Carreto, about 22 km northwest pf Puerto Obaldía Airport. All 10 occupants were killed. The crew was descending under VFR mode when the accident occurred for unknown reasons.

Circumstances:
At 0600LT, Aerotransportes Entre Rios Cargo Flight 501/90, crashed while attempting a night VMC takeoff from runway 27L at the Miami International Airport, Miami, Florida. The aircraft did not become airborne, and the pilot attempted unsuccessfully to reject the takeoff. The aircraft ran off the departure end of the runway and crashed on the west bank of a canal, about 960 feet from the departure end of the runway. Six of the ten persons aboard were killed. Two crewmembers and two passengers survived the accident. The aircraft was destroyed by impact and fire. The aircraft struck and destroyed an automobile; one occupant of the car was injured.
Probable cause:
The National Transportation Safety Board determines that the probable cause of the accident was an attempt to take off with an external makeshift flight control lock on the right elevator. The following findings were reported:
- An external elevator lock was carried on the aircraft. The use of this lock was not part of the certification of the aircraft.
- The external elevator control lock was installed on the right elevator after the aircraft landed at Miami and was still in place after the aircraft was loaded and taxied from the ramp for takeoff.
- The pilots could have detected the presence of this lock by referring to the flight control surface position indicators.
- The pilot was unable to rotate the aircraft to the takeoff attitude.
- By the time the pilot determined that he could not rotate the aircraft and initiated procedures to reject the takeoff, the aircraft had accelerated to a speed which prevented him from stopping on the runway.
- The aircraft struck a number of obstacles off the paved area of the airport; these impacts resulted in the destruction of the aircraft and fire.
