Circumstances:
The four engine aircraft departed Davao City Airport at 2051LT on a flight to Iloilo City where members of the Presidential Security Group should be picked up for Manila. During initial climb, the crew was cleared to climb to 18,000 feet when the aircraft entered an uncontrolled descent and crashed in the sea about 4 minutes after takeoff. The aircraft was destroyed and all 11 occupants were killed.

Parked at Manila-Ninoy Aquino Airport, the aircraft was ready for its flight to Iloilo City with 114 passengers and a crew of six on board. While being pushed back, the aircraft suffered three explosions and caught fire. 80 people escaped with minor injuries while 31 others escaped uninjured. Unfortunately, eight passengers died.
Probable cause:
It was determined that the center fuel tank had not been filled nor purged since March 9. It is possible that fuel vapors that accumulated in the central fuel tank ignited with the combination of high temperature (actual OAT 35° C) and a defect electric wire.

Circumstances:
While flying in poor weather conditions on a flight from Iloilo City to Manila, the airplane struck the slope of Mt Baco located in the occidental Mindoro province. The wreckage was found a week later, on the east slope of the mountain, at an altitude of 1,829 meters. All 33 occupants were killed in the crash. At the time of the accident, the airplane was off course by 51 km and weather conditions were poor with thunderstorm activity.
Probable cause:
The probable cause of the accident was a navigational error. Contributing factors were: adverse weather conditions and poor visibility, a 25-35 knot crosswind coming from the east, unreliability of the airborne navigational equipment due to either atmospheric disturbance night and terrain effects and/or its possible malfunctioning.

Circumstances:
Radio communications with the crew were lost while the aircraft was flying in heavy rain falls vertical to Negros Island. As it failed to arrive in Iloilo, SAR operations were conducted but eventually suspended few days later as no trace of the aircraft nor the 24 occupant was found. The wreckage was spotted by walkers seven months later in a mountainous and wooded area located east of Bacolod.
