Shortly after takeoff from RAF Butterworth, the left engine fire warning came on. The crew shot down the engine, feathered the propeller and obtained the permission to return for an emergency landing. On final, the airplane was too low, hit palm trees and crashed onto the runway. Four passengers were injured while 22 other occupants were unhurt. The aircraft was damaged beyond repair.

Circumstances:
The crew was performing a local training sortie at RAF Abingdon. After takeoff, the pilot shot down the left engine and feathered the propeller (simulation of an engine failure). Shortly later, the right engine lost power due to magneto issue. The crew then shot down the right engine and feathered the propeller as well. The crew elected to restart the left engine, without success. He then attempted an emergency landing in a prairie located near the airfield. While both pilots were unhurt, the aircraft was damaged beyond repair.
Probable cause:
Engine failure due to magneto issue.

After touchdown, the airplane went out of control, veered off runway and came to rest against a sand dune. While all crew members were unhurt, the aircraft was damaged beyond repair.
Probable cause:
Partial failure of the left engine after takeoff.

Shortly after takeoff from Singapore-Changi Airport, while in initial climb, the crew raised the landing gear when the aircraft went into a pitch up attitude. The pilot-in-command was able to regain control and attempted to ditch the aircraft into the sea, few hundred yards off shore. All three crew members were injured while the aircraft was lost.
Probable cause:
The elevators jammed shortly after liftoff for unknown reason.

En route, the crew encountered heavy turbulences. The aircraft went out of control, dove into the ground and crashed in a huge explosion in a prairie located near Loka. The aircraft disintegrated on impact and all four crew members were killed. Crew from the 78th Squadron.
Probable cause:
It was determined that the loss of control was probably caused by a structural failure that occurred on a wing as the aircraft was flying in heavy turbulences.

The pilot-in-command completed a belly landing and after touchdown, the aircraft slid for several yards before coming to rest. There were no casualties.

The aircraft was completing a flight from Luqa to RAF Dishforth with an intermediate stop in RAF Lyneham on behalf of the 242nd OCU. Shortly after takeoff, while climbing, the crew informed ground about an engine failure and elected to return for a safe landing when the aircraft stalled and crashed in a field. The airplane was destroyed and all ten occupants were killed, among them seven cadets.
Probable cause:
It was determined that an engine failed after takeoff and the crew shut down the wrong engine and feathered the wrong propeller. Due to loss of power, the aircraft stalled and crashed.

After touchdown, the aircraft encountered difficulties to stop within the remaining distance, overran and hit an embankment. On impact, the undercarriage were sheared off and the aircraft came to rest. There were no casualties.
