Circumstances:
The airplane departed Long Beach on a delivery flight to the Royal Canadian Air Force. Few minutes after takeoff, it suffered a structural failure, was abandoned by the crew and crashed east of Huntington Beach, about 22 km southeast of Long Beach Airport. The airplane was destroyed and all crew members parachuted to safety.

Circumstances:
Shortly after takeoff from Ambon-Laha Airport, while climbing, the airplane stalled and crashed into the sea few hundred metres offshore. Three crew members were killed and a fourth was injured.

While performing a practice bombing exercise, the crew encountered unknown technical problems and attempted to divert to Maryport Airport. On final approach, the twin engine aircraft went out of control and crashed few km from the airfield, bursting into flames. All four occupants were killed.

Circumstances:
The airplane departed Whenuapai Airport on a maritime patrol flight. While cruising at a height of 1,500 feet, the pilot-in-command initiated a left turn when the airplane stalled and crashed on a hill side near Helensville. The airplane was destroyed and all three crew members were killed.

Probable cause:
According to the RNZAF, weather conditions at the time of the accident were poor with atmospheric turbulences. The assumption that an engine failed was not ruled out. Following this accident, all RNZAF Lockheed Hudson were grounded to check all ailerons control systems.

The crew departed Nelson Airport on a three-hour survey flight within New Zealand. While cruising west of Nelson at night, the crew encountered poor weather conditions. In reduced visibility, the airplane impacted the slope of a mountain located near Westhaven Inlet. The wreckage was found seven days later and all four crew members were killed.
