The airplane departed Lille Airport to deliver equipment at Grójec-Słomczyn Airfield located south of Warsaw. While cruising in limited visibility due to marginal weather conditions, the airplane impacted the slope of Mt Jizerské located northeast of Liberec. All five crew members were killed.

While on a reconnaissance mission, the crew lost his bearings and became disoriented. Cruising in foggy conditions, the airplane impacted the slope of Mt Snæfugl. All four crew members were killed. The wreckage was found 11 days later by a local farmer.

Circumstances:
The airplane departed Lanvéoc-Poulmic NAS at 2100LT on May 11 on an anti-shipping operation over the Bristol Channel. While attacking at ship, the airplane was shot down by enemy fire. With the right engine on fire, the pilot reduced his altitude and ditched the airplane off Lundy Islands. The entire crew got into their dinghy and were rescued by a destroyed about six hours later. The airplane sank and was lost.
