Circumstances:
The aircraft was completing a cargo flight from Mys Schmidta to Novosibirsk with intermediate stops in Chokurdakh and Tiksi, carrying a load of 9,9 tons of various equipments. After takeoff from Tiksi, the engine n°4 failed. The captain decided to continue on three engines to Novosibirsk as a landing at Tiksi Airport was not possible due to poor weather conditions. Following an uneventful flight, the crew started the descent to Tolmachevo Airport when all three remaining engines lost power. The airplane lost height, struck trees and crashed in a wooded area located 13 km from the airfield. Four crew members were killed while two others were injured. The aircraft was destroyed.
Probable cause:
It was determined that the fuel was contaminated with salt water (sea water) which also caused a serious corrosion of the fuel supply system.

Circumstances:
Less than two minutes after takeoff from Novosibirsk-Tolmachevo Airport runway 07, while climbing by night at an altitude of 200 meters, the airplane entered clouds when the crew informed ATC about an in-flight fire. The captain was cleared to return for an emergency landing when control was lost 24 seconds later. The airplane entered a nose-down attitude and struck the ground at a speed of 335 km/h in an open field located 3 km from the airport. The aircraft disintegrated on impact and debris were scattered on a distance of 552 meters. All six occupants were killed. The airplane was carrying various prefabricated goods for a total weight of 11,278 kilos.
Probable cause:
It was determined that a fire broke out in the cargo compartment during the takeoff procedure but investigations were unable to establish the exact origin of the fire.

On final approach to Novosibirsk-Tolmachevo Airport, the crew encountered poor weather conditions and the captain decided to go around. A second attempt to land was made few minutes later. As the visibility seems to improve a bit, the captain decided to land. After touchdown on a waterlogged runway, the airplane was unable to stop within the remaining distance and overran. While contacting soft ground, the undercarriage were sheared off and the airplane came to rest. There were no casualties.
Probable cause:
The assumption that the accident was caused by an aquaplaning effect or an excessive speed upon landing was not ruled out.

The crew departed Novosibirsk-Severny Airport on a geophysical mission. En route, the right engine failed. Instead of feathering the propeller, the crew attempted few times to restart it but without success. Due to high drag, the airplane lost height and the speed decreased. The captain elected to make an emergency landing on a terrain located between the villages of Morozovo and Tavolgan. At a height of 330 meters, the different probes used for the mission were reinstalled in the cabin when the aircraft stalled and crashed in a wooded area. Five crew members were killed while three others were injured. The aircraft was destroyed.
Probable cause:
It was determined that the right engine failed because the fuel selector was connected to an empty fuel tank. Poor crew coordination and failure to follow the published procedures.

Circumstances:
On a night approach to Novosibirsk-Severny Airport, the crew encountered bad visibility due to fog. On final, the aircraft was 300 meters to the left of the glide and the crew failed to have a visual contact with the runway and ground. In such conditions, the captain decided to abandon the approach and made a go around. Few minutes later, the crew started a second approach with a horizontal visibility of 100 meters. Without any visual contact with the ground or the runway, the pilot-in-command continued the approach, passed below the decision height when the airplane hit trees and crashed in a wooded area located few km short of runway. The aircraft was totally destroyed upon impact but there was no fire. All 29 occupants were killed.
Probable cause:
It was determined that the visibility was below minima at the time of the accident. Because the crew failed to establish any visual contact with the ground and did not have the runway in sight, it would be necessary to divert to the alternate airport for a safe landing. Continuing the approach without any visual contact with the ground and passing below the decision height, the crew engaged the aircraft in a controlled flight into terrain.

Circumstances:
The crew left Novosibirsk-Severny Airport at 1400LT for a local training flight. At that time, weather conditions were poor with overcast at 200 meters, rain showers, visibility less than three km and icing conditions in clouds. While flying into the clouds at an altitude estimated between 1,200 and 1,250 meters with the left engine inoperative, the pilot-in-command lost control of the airplane that turned to the left, dove into the ground and crashed in flames few km from the airport. A crew member was injured while eight other occupants were killed.
Probable cause:
The exact cause of the accident could not be determined with certainty. However, it was reported that the left engine was inoperative at impact and that the propeller was feathered, a configuration apparently intended by the crew and part of the training program.

Due to the arrival of additional passengers coming from Yakutsk, a second flight was organized from Novosibirsk to Moscow. Snow falls all day and about 15 mm of ice accumulated on all aircraft. During taxiing, an additional layer of 8 to 10 mm of snow accumulated on the airplane. The takeoff run was considered as normal and after rotation, the airplane climbed to a height of about 100 meters and then stalled and crashed in a huge explosion about 900 meters past the runway end. The aircraft was destroyed by impact forces and a post crash fire and all 23 occupants were killed.
Probable cause:
It was determined that the aircraft was covered by ice and snow and that the following areas were polluted and contaminated by ice and snow: leading edges, trailing edges, ailerons, wings, fuselage, tail and stabs. In such conditions, the aircraft was unable to climb in a safely manner. The following findings were pointed out:
- The crew neglected some procedures referring to the flight preparation,
- The crew failed to check the aircraft prior to departure and failed to provide an appropriate deicing procedure,
- Poor flight preparation and organisation,
- Poor weather conditions,
- Lack of appropriate measures taken to clean the aircraft before takeoff.

Shortly after takeoff from Novosibirsk Airport, while climbing to a height of 50 meters, the right engine failed. The pilot in command elected to make an emergency landing but the aircraft was over a forest. It hit tree tops and eventually crashed in flames 250 meters further on. Two crew members were killed while three others were injured.
Probable cause:
Failure of the right engine during initial climb.

While descending to Novosibirsk Airport, the crew encountered bad weather conditions with thunderstorm activity, low clouds, poor visibility, hail and turbulences. On approach, lightning hit the cockpit. The copilot and the flight engineer were seriously injured and lost consciousness. The copilot fell onto the control column and the captain tried to maintain a directional control. Shortly later, the aircraft banked right to 65° and then dove into the ground and crashed in flames in a field located 12 km from the airport. The aircraft was destroyed by impact forces and a post crash fire and all 25 occupants were killed.
Probable cause:
The loss of control on approach was caused by adverse weather conditions resulting from a violent storm consisting of hail and turbulences. Investigations revealed that lightning struck the cockpit and that two crew members were seriously injured and lost consciousness, making it difficult to continue the flight. Investigators blamed the pilots who started the approach in such weather conditions but nevertheless hold that they were not properly briefed by meteorologists. It was concluded that the crash was the combination of the following factors: unexpected weather conditions, wrong decisions on part of the flying crew, poor management on part of the flight control service, lack of communication between the different departments involved and difficulties with radio contacts.

Just after liftoff, while climbing to a height of 2-3 meters, the aircraft stalled. As the undercarriage have been raised, the aircraft hit the ground on its belly, causing the propeller of the right engine to be sheared off. A blade went through the fuselage and penetrated into the cabin, killing a passenger. The aircraft slid for several hundred yards, lost its left wing and left engine before coming to rest 491 meters past the runway end. Five passengers were injured and the aircraft was written off.
Probable cause:
The accident was attributed to a lack of coordination within the flight crew. It was determined that the flight engineer mistakenly reduced the engine power shortly after liftoff, causing the speed to drop and the aircraft to stall. The captain's lack of training was considered as a contributory factor.
