Circumstances:
The pilot reported, and airport security video confirmed, that during a takeoff attempt, the right wing contacted the runway and the pilot pulled back excessively on the yoke. The airplane pitched up, stalled, and descended back on to the runway. It subsequently traveled off the end of the runway and impacted trees, before coming to rest on its side. The pilot added that in retrospect, he should have rejected the takeoff when the right wing contacted the runway. Examination of the wreckage by a Federal Aviation Administration inspector did not reveal any preimpact mechanical malfunctions, nor did the pilot report any. The inspector noted that both wings separated, and the fuselage was substantially damaged.
Probable cause:
The pilot’s failure to maintain aircraft control during a takeoff attempt, which resulted in an aerodynamic stall, runway excursion, and collision with trees.

Shortly after takeoff from Juba Airport runway 31, while climbing, the crew encountered an unexpected situation and was forced to attempt an emergency landing. The airplane crash landed in an area located in the Hai Referendum district about 3 km northwest of the airport. All five occupants escaped with minor injuries while the airplane was severely damaged.

The single engine airplane departed Buochs Airport on December 12 on a delivery flight to its new owner, Smart Cakrawala Aviation based in Indonesia. The crew made a stop in Maribor and Podgorica then continued to Heraklion. On December 15, shortly after takeoff from Heraklion-Nikos Kazantzakis Airport runway 09, en route to Hurghada, the airplane climbed to an altitude of 1,400 feet when the pilot reported problems and declared an emergency. After completing a right turn to return to the airport, the airplane lost height and speed then stalled and crashed into the sea off Karteros. The pilot aged 26 was slightly injured while the passenger, an Indonesian citizen aged 68, died shortly later. The airplane floated for a while then sank. Named 'Franz', the airplane was the last PC-6 built by the Swiss manufacturer.

The twin engine airplane was chartered by the Grupo San Germán Express to carry a team of six people from Medellín to Pizarro, Chocó. Shortly after takeoff from runway 02 at Medellín-Enrique Olaya Herrera Airport, while in initial climb, the crew encountered engine problems. The airplane went out of control and crashed onto a house located in the district of Belén Rosales, less than 500 metres from the runway end, bursting into flames. The house and the aircraft were destroyed and all 8 occupants were killed.

Circumstances:
The airplane was on a schedule service (flight LA2213) from Lima to Juliaca. On board were 102 passengers and a crew of six. During the takeoff roll on runway 16L, after a course of 1,200 metres and at a speed of 131 knots, the aircraft collided with a fire truck that entered the active runway from the right, at taxiway Bravo. A team of two heavy fire trucks and one light support vehicle were engaged in a 'Response Time' exercise that started at 1510LT, one minute prior to the accident. The goal of this exercise was to test the capacity of the new fire station. The first fire truck entered the active runway while the airplane was accelerating for takeoff. Upon impact, the right main gear and the right engine were torn off. The airplane caught fire and slid for about 1,600 metres then veered to the right and came to rest, bursting into flames. Forty passengers were injured, among them four seriously. The airplane was heavily damaged by fire and written off. Among the personnel in the fire truck, two were killed and a third was seriously injured.
Probable cause:
It was determined that the crew of the fire truck entered the active runway 16L without prior permission from air traffic controller. This chain of events was caused by the lack of joint planning, poor coordination and the non-use of ICAO-standardized communication and phraseology.
