While flying at low height, the pilot lost control of the airplane that crashed near Kalinin (now Tver). The aircraft was destroyed and the pilot was injured.

Probable cause:

Loss of control due to lack of discipline on part of the pilot.

The airplane departed Chkalovski Airport in Moscow on a round flight via Monino, Shchelkovo, Verekleevka, Oryol and Kharkiv, carrying four crew members and a load of 1,740 kg of ammunition. Ten minutes after the engine were started, the crew initiated the takeoff procedure. After liftoff, the airplane encountered difficulties to gain height, impacted trees and crashed in a wooded area, bursting into flames. All four occupants were injured and the aircraft burnt out.

Probable cause:

It was determined that the takeoff procedure was precipitated and that the period of time of 10 minutes between the engine startup and the takeoff was insufficient.

The airplane was en route from Belarus to Smolensk, carrying one passenger, five crew members and a load of brochures that should be distributed in Smolensk. Approaching Smolensk from the southeast, the airplane was mistakenly shot down by the Soviet Flak (ground artillery). The pilot reduced his altitude and attempted an emergency landing. The airplane crash landed in a rough terrain and came to rest. All six occupants escaped uninjured and the airplane was damaged beyond repair.
