Circumstances:
The crew was completing a local training mission at Vitoria Airport. In good weather conditions, the crew was approaching the airport in a flapless configuration when an alarm sounded in the cockpit. The crew turned off the circuit breaker so the alarm stopped. The aircraft landed on its belly and slid for few dozen metres before coming to rest. All four occupants escaped uninjured and the aircraft was damaged beyond repair.
Probable cause:
The aircraft belly landed after the crew failed to follow the approach checklist and turned off the circuit breaker.

Circumstances:
The aircraft was flying the Rio de Janeiro - Vitória segment of a scheduled international flight. A 2220 GMT it reported it was over Guararapi at 2,700 m and in IMC. The aircraft was authorized to descend to 2,100 m and told to maintain that altitude until reaching the Vitória NDB. At 2228 it reported it was three minutes out at 2,100 m and in VMC. The flight continued its descent and was given landing instructions for runway 23. The controller in the tower watched the aircraft descending, and at the end of the downwind leg he saw the landing lights being adjusted. When the aircraft reported on final, the landing instructions were repeated. Shortly thereafter power was applied in an effort to climb the aircraft, but it collided with a eucalyptus tree at a height of 40 m, 1,860 m from the threshold of runway 23. At that stage of the approach the aircraft should been at a minimum altitude of 190 m (150 m above the ground). Fire broke following impact and destroyed the airplane. Two passengers were seriously injured while all 23 other occupants were killed.
Probable cause:
The pilot did not carry out the approach in accordance with the procedures prescribed by the airline and misjudged his distance from the runway.

Circumstances:
The crew consisting of three engineers and three pilots was involved in a training flight. While flying off Vitória, the airplane went out of control and crashed in unknown circumstances into the sea. SAR operations were conducted but eventually suspended few days later as no trace of the aircraft nor the six crew members was found.

The aircraft named 'Baby Clipper' was circled over Rio and was making a normal approach to the seaplane landing area adjoining the air line's Rio base near Santos Dumont Airport, in accordance with the company's established operating procedure, when it suddenly lost power on the left engine, yawed to the left, and started a descending turn in the same direction. The airplane continued to lose altitude and to turn at a sharper and steeper angle until it struck a caisson anchored at right angles to a small island in the harbor immediately adjacent to its landing approach path. All four members of the crew and 10 passengers were fatally injured, one passenger escaping with serious and another with minor injuries.

Probable cause:
Ample evidence was obtained during the Air Safety Board's investigation of the accident that the left engine suffered a sudden loss of power at a critical time during the landing approach, although, since available evidence failed to supply any conclusive explanation for this loss of power, and a detailed examination of the engine, after disassembly, revealed no indication of structural failure or mechanical defects in flight, the report stated that 'the cause of the loss of power from the left engine is unknown'. In conclusion, the accident was caused by a sudden loss of power from the left engine during the landing approach, necessitating an attempted landing under extremely hazardous conditions.

While descending to Rio de Janeiro seaplane base in poor weather conditions (heavy rain falls and strong winds), the three engine aircraft named 'Marimba' hit the slope of a mountain located in the Serra do Sambê, some 60 km northeast of Rio de Janeiro. All 10 occupants were killed.
Probable cause:
Controlled flight into terrain while descending in poor visibility due to heavy rain falls. High winds may have been a contributing factor.
