Circumstances:
On a flight from Ulan-Ude to Nizhneangarsk, while descending to an altitude of 3,050 metres about 30 km from the destination airport, the crew contacted ATC and reported the failure of the left engine. The approach was continued to runway 22. After touchdown, the crew started the braking procedure when the airplane deviated to the right then veered off runway. It rolled in a grassy area, cwent through the perimeter fence and eventually impacted the building of a sewage treatment plant located 380 metres to the right of the runway centerline. The captain and the flight engineer were killed and the copilot was seriously injured. 10 other occupants were injured. The aircraft was destroyed by a post crash fire.

Circumstances:
Following an uneventful flight from Ulan-Ude, the crew initiated the approach to Yakutsk-Platon Oyunsky Intl Airport Runway 23L. Due to work in progress, the runway 05R threshold was displaced by 1,150 metres, reducing the landing distance to 2,248 metres for the runway 23L on which the touchdown zone lighting system was unserviceable. On approach, the crew was informed by ATC that the friction coefficient was 0,45. The crew completed the landing on runway 23L with a slight tailwind component of 4 knots and started the braking procedure. Unable to stop within the remaining distance, the aircraft overran, entered the construction area and collided with the junction of a concrete section under reconstruction, causing both main landing gear to collapse (the left main gear was torn off). The aircraft slid for another 250 metres before coming to rest. All 92 occupants evacuated safely and the aircraft was damaged beyond repair.
Probable cause:
The cause of the aviation accident with the RRJ 95B RA-89011 aircraft when landing at night with one deactivated thrust reverser was the transfer to the crew of incorrect information about the value of the friction coefficient, which led to the landing on an icy runway, the average normative friction coefficient which was less than 0.3, which did not allow landing in accordance with current regulations. This led to a significant increase in landing distance, rolling the aircraft out of the runway and collision with the junction of the reconstructed section of the runway with a height of about 0.4 m, which led to the destruction of the main landing gear supports and damage to the engines with fuel leakage without causing a fire.
The contributing factors were the following:
- Absence of a connecting ramp (which was not envisaged by the reconstruction project) between the current and the part of the runway being reconstructed;
- inefficiency of the SMS of Yakutsk Airport JSC in terms of identification and control of risks associated with the possibility of ice formation at the runway, and insufficient control over the implementation of the SMS by aviation authorities;
- erroneous determination of the Xc value at the last measurement;
- inefficiency of ice removal procedure during runway cleaning due to lack of chemical reagents for ice removal and/or thermal machines at the airfield;
- lack of information from the RRJ-95LR-100 RA 89038 aircraft crew about actual rolling out of the runway, which was an aviation incident and was subject to investigation;
- failure by airport and ATC officials to take appropriate action after receiving the RRJ-95LR-100 RA-89038 low friction report from the aircraft crew.

On final approach by night to Ulan Ude-Mukhino Airport, the aircraft struck the ground 300 meters short of runway, slid for few dozen meters and came to rest, broken in two and bursting into flames. All six crew members were rescued while the aircraft was destroyed.

The crew was completing a local training flight at Ulan-Ude Airport. While circling in the vicinity of the airport, the crew lost control of the airplane following a failure of all four engines. The aircraft stalled and crashed in a field located 4 km from the airport. All six crew members were killed.
Probable cause:
Failure of all four engines due to a fuel exhaustion.

The airplane departed Ulan-Ude at 0700LT on a cargo flight to Ufa with intermediate stops in Krasnoyarsk, Novosibirsk, Omsk, Petropavl, Kurgan and Chelyabinsk, carrying five crew members, three passengers and a load of 663 kg of various goods. About 20 minutes after takeoff, while cruising at an altitude of 2,400 meters, a passenger announced to the crew that a fire erupted in the rear part of the cabin. Smoke spread in the cabin and the cockpit and as the crew was unable to extinguish the fire, the captain decided to reduce his altitude and attempted an emergency landing in a prairie. The aircraft crash landed and came to rest in flames in an open field located 4 km from the village of Kharat. All eight occupants escaped while the aircraft was partially destroyed by fire. 40 hours later, a female passenger died from her injuries.
Probable cause:
The aircraft was carrying several wooden crates containing bottles of 20 liters of flammable products placed on a carpet of wood chips. It is believed that these boxes had been used many times in the past but had not been cleaned and the wood chips were soaked with flammable products, maybe because some of the bottles were not properly sealed. In flight, the hot air pulsed into the cabin via the heating system caused the wood chips to burn and the fire could not be extinguished by the crew.

The crew departed Ulan-Ude-Mukhino Airport on a local training flight. Following several circuits, it was decided to shut down an engine to simulate a failure. After the propeller was feathered, the flight engineer mistakenly shut down the second engine. The crew completed a belly landing on the Bodorodski Island located about 8 km short of runway 26. The aircraft was partially consumed by a post crash fire and all crew members were slightly injured.
Probable cause:
Engine mistakenly shot down by the flight engineer during a single engine exercise.

The aircraft departed Tashkent on a cargo flight to Yuzhno-Sakhalinsk with intermediate stops in Fergana, Novosibirsk, Krasnoyarsk and Irkutsk, carrying a load of 10,475 kilos of grapes. En route from Krasnoyarsk to Irkutsk, the crew was informed about poor weather at destination and decided to divert to Ulan Ude although that airport was not suitable for such type of aircraft. While approaching from the north in marginal weather conditions, at an altitude of 1,200 meters, the airplane struck the slope of a wooded mountain located 32 km north of the airfield. The aircraft was destroyed and all eight occupants were killed.
Probable cause:
Controlled flight into terrain resulting from a crew error who mistakenly imputed wrong QNH setting for the approach. Lack of visibility and poor flight preparation were considered as contributing factors.

The single engine airplane departed Irkutsk Airport on a cargo flight to Mogocha, carrying one passenger, one pilot and a load of 230 kg of various goods. En route, the engine caught fire an the pilot attempted an emergency landing when the airplane crashed on the ice of Lake Baïkal, some 10 km west of Posolskoye. SAR operations were initiated three days later only. The aircraft was totally destroyed by a post crash fire and both occupants were not found. Some foot print were found on ice and snow but both occupants were later considered as deceased.
Probable cause:
Engine fire in flight, maybe following the failure of a fuel supply or a malfunction of the carburetor.
