Circumstances:
On the leg from Kazan to Sverdlovsk-Koltsovo Airport, while cruising at an altitude of 1,200 meters, both engines failed simultaneously. The pilot-in-command reduced his altitude and attempted an emergency landing when the airplane struck trees and crashed in flames in a wooded area located about 120 km west of Sverdlovsk. The wreckage was found the following morning at 0800LT. All five crew members and four passengers were killed while 17 other occupants were injured. The aircraft was destroyed.
Probable cause:
In-flight failure of both engines due to a fuel exhaustion. This was the result of a poor flight preparation on part of the crew who failed to fuel the aircraft with a sufficient quantity prior to departure. It was determined that a total of 1,108 kg of fuel was in the tanks, about 550 kg less than the necessary amount for this routing.

The aircraft departed Izhevsk Airport at 0220LT bound for Sverdlovsk and continued at an assigned altitude of 2,400 meters. On final approach to Koltsovo Airport, at a height of 250 meters, the right engine failed. The airplane lost height, struck trees and crashed. Both pilots were killed while all three other occupants were injured. The aircraft was destroyed.
Probable cause:
Failure of the right engine because it was connected to a empty fuel tank. Poor flight supervision on part of the flying crew and failure of the crew to switch the fuel selector on a suitable tank in due time.

One minute after takeoff from Sverdlovsk-Koltsovo Airport, while in initial climb, the airplane banked left and right and encountered difficulties to gain height when it stalled and crashed 250 meters past the runway end. The wreckage was spotted 150 meters to the left of the runway centerline. Both pilots and three passengers were killed while ten other occupants were injured. Two weeks after the accident, a passenger died from his injuries.
Probable cause:
It was determined that the flaps were deployed at 25° for takeoff, too much for this procedure. Thus, the aircraft's speed after rotation was too low, deteriorating its stability and its lateral control. Investigations were able to determine that the flying crew failed to identify and eliminate the abnormal behavior of the aircraft during initial climb, probably because his attention was diverted. This situation led the aircraft to stall and the distance between the aircraft and the ground was insufficient to expect a recovery. At the time of the accident, the crew was on duty since 13 hours and 40 minutes and the day before, it observed a ON duty period of 17 hours. Thus, it is believed that fatigue accumulated during these two consecutive days, which could be considered as a contributing factor.

The airplane departed Omsk Airport with a delay of two hours. En route, weather conditions deteriorated and the visibility was poor due to snow falls. Despite the situation, the crew decided to continued to Sverdlovsk. Approaching the destination, the crew were unable to locate the airport and in low visibility, attempted to land when the airplane impacted a hill, bounced and crashed 70 metres further. The wreckage was found nearby the Verkh-Isetskiy Lake, west of Sverdlovsk. All three crew members were injured and the mechanic later died from injuries sustained.
Probable cause:
Controlled flight into terrain after the decision of the crew to continue in poor visibility due to adverse weather conditions.

Crash of a Tupolev ANT-40 near Sok-Karmala: 1 killed

When weather conditions improved, the crew was cleared for takeoff and departed Kuybyshev Airport at 1127LT on a flight to Sverdlovsk. The pilot continued the flight at an altitude of 50 metres and once he arrived over the village of Sok-Karmala, he encountered foggy conditions. In low visibility, the airplane impacted trees, lost height and crashed 135 metres further, bursting into flames. The flight engineer was seriously injured and the pilot was killed.
Probable cause:
Failure of the pilot who continued the flight at an unsafe altitude in insufficient visibility caused by foggy conditions until the airplane collided with obstacles.

The airplane departed Tyumen Airport at 0805LT on a special flight to Sverdlovsk, carrying three crew members and seven passengers on their way to a political party committee meeting. A little more than an hour into the flight, while cruising at an altitude of 100 metres in good weather conditions, the left engine failed. The pilot lost control of the airplane that rolled to the left then entered a dive and crashed 1,5 km from the village of Chernokorova, bursting into flames. The airplane was totally destroyed and all 10 occupants were killed.
Probable cause:
Loss of control in flight following the failure of the left engine for undetermined reasons.
The following contributing factors were identified:
- The pilot took the decision to continue the flight at an unsafe altitude of 100 metres above ground,
- The crew failed to proceed with a pre takeoff check of the airplane,
- No technical check of the airplane was made since last October 4,
- The total weight of the aircraft was 160 kg above MTOW.

The crew departed Yanaul at 1000LT on a schedule service to Sverdlovsk. En route, weather conditions worsened and the visibility was very low due to thick fog. Approaching the village of Ananino, the airplane passed over houses at a height of about 4 metres. Then the pilot initiated a turn when the airplane stalled and crashed in a field outside the village. The airplane was destroyed and the mechanic was killed. Seriously injured, the pilot was transferred to a local hospital but he died few hours later.
Probable cause:
It was determined that the accident was the consequence of a stall during a turn initiated at low height because the airplane' speed was too low. The Chief of the Yanaul Airport authorized the pilot to start the flight despite the weather conditions en route were considered as poor. Also, the weather bulletins transmitted to the crew were incorrects and conflicting.
