The airplane departed RAF Stradishall at 2000LT on an operation to Hamburg. While passing over the Netherlands at night, it was shot down by the pilot of a Me.110 and crashed in Ter Apel, near the German border. Five crew members were killed and a fifth became PoW.

Circumstances:
The airplane departed RAF Stradishall at 1825LT on an operation to Düsseldorf. While returning to base in the evening, the crew declared an emergency due to engine failure but landing permission was denied as another Wellington was obstructing the runway. From a height of 150 feet, the captain initiated a go around procedure when the airplane stalled and crashed shot of runway. All six crew members were injured.

Just after a night takeoff from RAF Stradishall, the left engine failed. The airplane sank back onto the runway and came to rest, bursting into flames. All six crew members were quickly rescued and the aircraft was destroyed by fire.
