Circumstances:
The twin engine aircraft departed a private strip in Itapuranga on a flight to Goiânia, carrying seven passengers and two pilots. About 10 minutes after takeoff, while in cruising altitude, the captain informed ATC about the failure of the right engine. Because the flight was short (25 minutes in total), he decided to continue to Goiânia. Twelve minutes later, the left engine failed as well. The crew declared an emergency and as he was unable to reach Goiânia Airport, he attempted an emergency landing in a pasture. The aircraft crash landed 13 km short of runway 14 threshold. All nine occupants were rescued, among them seven were injured, three seriously.
Probable cause:
Both engines failed 12 minutes apart due to fuel exhaustion. The following contributing factors were identified:
- Fuel tanks were empty,
- The fuel quantity was insufficient to cover the requested flying distance,
- Poor flight preparation and planning,
- Complacency on part of the copilot,
- Wrong decisions on part of the captain,
- Failures in the organization of flights within the operations of the Government of the State of Goiás,
- Miscalculations in fuel consumption.
- Operating an airplane with faulty fuel gauges,
- Failures in monitoring related to flight safety culture,
- Poor crew resources management,
- Poor crew coordination,
- Lack of crew interactions.

Circumstances:
Following an uneventful flight from Goiânia, the crew started the descent to Belém-Val de Cans Airport runway 06 via heading 243° then reported 6 nm from the airport at an altitude of 2,000 feet. Eleven seconds later, the aircraft went out of control and crashed in a wooded area located on the Oncas Island, few km short of runway 06 threshold. The aircraft was totally destroyed upon impact and all four occupants were killed.
Probable cause:
It is believed that the loss of control may have been the result of windshear that existed in the approach path of runway 06.
The following contributing factors were reported:
- The crew did not have sufficient training and experience to manage a windshear situation and thus, was unable to take the appropriate corrective actions,
- Poor crew coordination,
- The captain's experience on this type of aircraft was low,
- Poor supervision,
- Lack of safety culture by the operator.

Circumstances:
On the leg from Rio Verde to Goiânia, the crew informed ground that a fire erupted in the cabin and the captain decided to divert to Palmeira de Goiás for a safe landing. The crew tried several times to extinguish the fire but without success. Eventually, the pilot-in-command reduced his altitude in an attempt to make an emergency landing when, at a height of about 20 meters, the airplane exploded in the air and crashed few km from the airport of Palmeira de Goiás. The aircraft disintegrated on impact and all 24 occupants were killed.
Probable cause:
Investigators arrived on scene two days after the crash and confirmed that the wreckage was looted by locals. Due to the absence of evidence, it was very difficult to establish the exact cause of the accident. Nevertheless, it is believed upon the testimony of several eyewitnesses that a fire erupted in the rear of the cabin and not in the fuel tanks located in the wings. It is possible that the fire was caused by inflammable materials stored in the bagage compartment. The fact that the aircraft was not equipped with fire detectors was considered as a contributory factor as it took time for the crew to analyse and understand the situation.

Crash of a Douglas DC-3-393 in Uberlândia: 9 killed

Circumstances:
On final approach to Uberlândia Airport, the airplane was too low and hit trees with its left wing, stalled and crashed. Six passengers and three crew members were killed while 22 other occupants were injured. The aircraft was destroyed.
Probable cause:
The pilot-in-command decided to continue the approach below the minimum descent altitude, neglecting the published procedures.
