Circumstances:
The twin engine airplane was carrying two technicians and two pilots bound for a mine field located near Lero, Kankan. It made an intermediate stop at Conakry on a flight from Sal, Cape Verde. En route, the crew encountered poor weather conditions with low clouds and fog when the airplane struck the slope of a mountain and disintegrated on impact. All four occupants were killed.

Circumstances:
The aircraft was carrying a delegation of Senior Officials of the Guinea Air Force to Monrovia. On final approach to Roberts Airport, the aircraft descended too low, impacted trees and crashed in a wooded area located near Charlesville, some 4 km short of runway. The aircraft was destroyed by impact forces and a post crash fire and all 11 occupants were killed, among them General Souleymane Kéléfa Diallo, Chief of Staff of the Guinea Army Forces. The delegation was on its way to Monrovia to take part to the celebration of the Liberia Army forces anniversary.
Probable cause:
In July 2013, the investigation board confirmed that the accident resulted of multiple errors committed by the pilots who did not carry sufficient attention to the flight and the approach procedure. Investigators also concluded that the flight crew was tired, which was considered as a contributing factor because their faculties and capacities were diminished.

The aircraft departed Nouakchott in he evening of July 27 on a regular schedule service to Conakry with an intermediate stop in Dakar, carrying 91 passengers and a crew of six. Following an uneventful flight from Dakar, the crew started a night approach to Conakry-Gbessia Airport. After touchdown on a wet runway (due to recent rain falls), the aircraft was unable to stop within the remaining distance. It overran, collided with the ILS antenna and some approach lights, lost its nose gear and came to rest 500 metres further. Ten passengers were injured while all 87 other occupants escaped unhurt. The aircraft was damaged beyond repair.
Probable cause:
Wrong approach configuration on part of the crew, causing the aircraft to land half way down the runway and reducing the landing distance available. The crew failed to follow SOP's and to initiate a go-around procedure as the landing was obviously missed.

Circumstances:
Due to poor weather conditions in Freetown, the flight was delayed for hours. Finally, the crew was cleared for takeoff from runway 12/30 which is 3,200 metres long. During the takeoff roll, the captain decided to abort for unknown reasons. Unable to stop within the remaining distance, the aircraft overran, lost its right main gear and right engine before coming to rest in a grassy area located 100 metres past the runway end. All 127 occupants were rescued, among them 50 passengers were injured. The aircraft was damaged beyond repair.
Probable cause:
The accident was the consequence of the non-application by the crew of the checklist prior to takeoff and the lack of coordination between the copilot, who was the pilot flying, and the captain.

While descending to Conakry-Gbessia Airport, the crew encountered technical problems with the undercarriage that could not be lowered. Several attempts were made to lower the gear manually but this was unsuccessful. The crew decided to complete a belly landing. After touchdown, the aircraft slid for few dozen metres and came to rest on the runway. All 50 occupants evacuated safely and the aircraft was damaged beyond repair.

During the takeoff roll at Monrovia-Roberts Intl Airport, at V2 speed, the captain elected to takeoff but the aircraft failed to rotate. Takeoff was rejected and the crew initiated an emergency braking procedure. Unable to stop within the remaining distance, the aircraft overran at a speed of 200 km/h and collided with a drainage ditch. The nose gear was torn off while the right main gear collapsed. The aircraft came to rest 487 meters past the runway end and was damaged beyond repair. All six crew members escaped with minor injuries.
Probable cause:
It was determined that the ground personnel at Monrovia Airport failed to follow the agreement signed with Aeroflot regarding ground handling, export cargo transportation and calculation. At the time of the accident, the total weight of the aircraft was 6,604 kilos above MTOW and the CofG was 18% too far forward the permissible value. In such conditions, the aircraft was unable to takeoff.
