Circumstances:
The aircraft departed Rome-Ciampino Airport on an ambulance flight to Cagliari, carrying three pilots, three doctors and a cooler containing a heart for a patient. The descent to Cagliari-Elmas Airport was initiated by night under VFR mode. After the crew was cleared to descend to 2,500 feet, ATC reported runway 32 in use and asked the crew to report on short final. About two minutes later, at an altitude of 3,333 feet, the aircraft struck the slope of Mt Su Baccu Malu located 32 km northeast of Cagliari Airport. The aircraft was totally destroyed by impact forces and all six occupants were killed.
Probable cause:
The accident, classified as CFIT, was caused by the conduct of the flight at a height significantly below the Area Minimum Altitude, insufficient to maintain the separation from the ground during a night visual approach in the absence of adequate visual reference.
possible contributory factors that have been identified:
- The aircraft instrumentation did not include a GPWS or TAWS, whose installation is not required by law;
- The erroneous descent by visual flight references, confusing the Elmas runway lights, given that the crew had no special familiarity with the area of Cagliari, the onset of a perspective illusions phenomena, with specific reference to the so-called "black hole approach";
- The misunderstanding by crew members, of the Cagliari Approach controllers instruction to transfer to Elmas TWR ('CIT 124 continue not below 2500 feet, further descent with Elmas TWR 120.6 bye') which may have created the impression, despite the crew had confirmed that they are able to separate themselves from the obstacles that the descent down was free of obstructions;
- Failure to use published procedures and available instruments in a descent to a closer airport and in an unfamiliar area , under conditions of total darkness;
- The anticipation of the deviation from the airway perhaps caused [the crew] to try to speed up the arrival at destination, which determined overflying areas of higher elevation;
- Read errors of the elevations listed in the maps consulted, facilitated by the non representation of the ground color;
- The extended period of wakefulness without adequate rest, which may have contributed to a reduction in the performance of the crew.

The airplane went out of control and crashed on the slope of Mt Punta Tricoli located in the region of Gairo Sant'Elena. The aircraft was destroyed and all three occupants were killed.

Few minutes after its takeoff from Cagliari-Elmas Airport, while climbing, the airplane went out of control, dove into the ground and crashed on the Mt Sinnai located 15 km northwest of the airfield. The aircraft was destroyed by impact forces and all 19 occupants were killed.
Probable cause:
It was determined that the loss of control was caused by the failure of the left wing which detached in flight from its root. It is believed that the loss of the left wing was caused by a structural failure due to the wrong attitude of the aircraft. Nevertheless, investigations were unable to establish if this was the result of a fault on part of the flying crew or if the aircraft encountered strong downdrafts or turbulences.

The aircraft left RAF Pershore at 0330LT on a flight to Karachi with a planned stop in Cagliari and Castel Benito. While flying over Germany, the crew encountered marginal weather conditions (dark low clouds, heavy rain, thunder and lightning) when the aircraft was attacked by several German fighters. The Dakota went out of control, dove into the ground and crashed in a pasture. All 20 occupants were killed, among them several RAAF staff bound for Karachi.

Shortly after take off from the Lido di Ostia seaplane base, while climbing to a height of some 300 feet, the crew encountered a loss of power on one engine and the captain decided to return. On approach, after passing over the Tiber Estuary, the floatplane encountered atmospheric turbulences, stalled and crashed. Five occupants were killed. The airplane was operated on behalf of Nucleo Comunicazioni Ala Littoria with the military registration MM60419.

Engine problems.

The three engine float plane departed Cagliari in the early morning and continued its route along the east coast of Sardinia. While overflying over the Tyrrhenian Sea, the aircraft entered an uncontrolled descent and crashed. The crew did not send any mayday message. Debris were found floating on water some 145 km east of Olbia and none of the 20 occupants survived the crash.

Circumstances:
While landing in the harbor of Olbia, on a very calm sea, the seaplane landed hard, overturned and came to rest upside down off Terranova Pausania. A passenger was killed while 11 other occupants were injured. The aircraft was damaged beyond repair.
Probable cause:
The accident was caused by an optical illusion that suffered the pilot-in-command prior to landing. Due to the fact that the sea was very calm, in conjunction with the light density, the pilot misjudged the distance between the seaplane and the water surface, causing the aircraft to slam the water.
