Circumstances:
The airplane was engaged in a local practice flying exercise out of RCAF Dartmouth for qualification first pilot. After takeoff, at a height of 100 feet, the right engine cut out and the airplane crashed into water off the McNabs Island, about five miles southwest of the airbase. Both crew members were killed.

Probable cause:
Failure of the main fuel line on lower left hand side of the right engine. Failure of the left engine due to wrong manipulation of the fuel selector valve. The pilot or crewman turned the engine fuel selector valve to the 'right engine ON' position, thereby cutting off the fuel supply to the good motor (left) and diverting it to the dead motor. This caused the good motor (left) to fail also.

The airplane departed RAF Thornaby on a maritime patrol flight. On return, it collided with the cable of a barrage balloon, went out control and crashed in a field, bursting into flames. All four crew menbers were killed.

The twin engine airplane departed Sumburgh on a maritime patrol flight. It was shot down by the pilot of a Me.110 and crashed into the sea off the Shetland Islands. All four crew members were killed.

Circumstances:
The crew (one instructor and one student pilot) was performing a training flight. During the last segment, just prior to touchdown, the aircraft stalled and hit violently the runway surface. All landing gears were sheared off and the aircraft slid for several yards before coming to rest. Both crew members were uninjured while the aircraft was damaged beyond repair.

Circumstances:
During the takeoff run, the aircraft swerved, went out of control, veered off runway and came to rest in the adjacent grassy area, bursting into flames. Both pilots escaped unhurt while the aircraft was destroyed by fire.
Probable cause:
It was determined that a landing gear sank during the takeoff run and punctured a fuel tank, causing a fire that could not be extinguished in time.
