The aircraft was on its way from Moscow to Chelyabinsk, carrying a crew of six and 13 members of the ice hockey team of the VVS Moscow (Ice Hockey Branch of the Army) who should play against the Dzerzhinsky club of Chelyabinsk. Upon arrival in Chelyabinsk, the crew encountered poor weather conditions and as it was not possible to land, the captain decided to divert to Sverdlovsk. Unfortunately, while approaching Sverdlovsk, weather conditions deteriorated and the visibility was poor due to snow falls. Four times, the crew tried to land but was forced to go around. During the fifth attempt to land, the aircraft was too low and crashed in a field few hundred yards short of the runway threshold. The aircraft was destroyed and all 19 occupants were killed.

Few minutes after takeoff from Sverdlovsk-Koltsovo Airport, while climbing, the crew encountered severe icing conditions and the windshield went covered by ice. The pilot in command lost control of the aircraft that banked right and crashed right wing first in a field located 8,5 km southeast of the airport, near the Iset River. Two crew members and the passenger were killed while three other occupants were injured.
Probable cause:
The loss of control was the result of a series of mistakes on behalf of the crew who adopted poor flight techniques after departure. At the time of the accident, weather was marginal with severe icing conditions. It was stated that the crew was inexperienced, especially the copilot, which remains a contributing factor. The inefficiency of the windshield deicing system and a lack of crew training were also considered as contributing factors.

The crew left Verknyaya Salda in the morning on a cargo flight to Ufa. En route, he encountered poor weather conditions and the visibility was reduced by snow falls. As the crew did not have the appropriate charts for the region, he obtained the assistance of the ATC and was vectored to Tyumen Airport. Few minutes later, while cruising at an assigned altitude of 2,100 meters, the central engine lost power and failed. Shortly later, both other engines (left and right) lost power and produced black smoke. In such conditions, the crew was unable to maintain a safe altitude and the captain decided to make an emergency landing. The aircraft hit tree tops and crashed in a wooded area located 14 km north of Mezenka. Four crew members were injured while the captain was killed.
Probable cause:
It was determined that all three engines lost power and eventually failed due to an excessive accumulation of wet snow in the carburetor and the air intake.

Just after liftoff from Sverdlovsk-Koltsovo Airport, while climbing, the left engine failed. The pilot completed an emergency landing in a swampy area near the airport. While all five crew members were uninjured, the aircraft was later struck of charge as no spare parts were available.
Probable cause:
Failure of the left engine after takeoff.

The airplane departed Omsk Airport with a delay of two hours. En route, weather conditions deteriorated and the visibility was poor due to snow falls. Despite the situation, the crew decided to continued to Sverdlovsk. Approaching the destination, the crew were unable to locate the airport and in low visibility, attempted to land when the airplane impacted a hill, bounced and crashed 70 metres further. The wreckage was found nearby the Verkh-Isetskiy Lake, west of Sverdlovsk. All three crew members were injured and the mechanic later died from injuries sustained.
Probable cause:
Controlled flight into terrain after the decision of the crew to continue in poor visibility due to adverse weather conditions.

The airplane departed Tyumen Airport at 0805LT on a special flight to Sverdlovsk, carrying three crew members and seven passengers on their way to a political party committee meeting. A little more than an hour into the flight, while cruising at an altitude of 100 metres in good weather conditions, the left engine failed. The pilot lost control of the airplane that rolled to the left then entered a dive and crashed 1,5 km from the village of Chernokorova, bursting into flames. The airplane was totally destroyed and all 10 occupants were killed.
Probable cause:
Loss of control in flight following the failure of the left engine for undetermined reasons.
The following contributing factors were identified:
- The pilot took the decision to continue the flight at an unsafe altitude of 100 metres above ground,
- The crew failed to proceed with a pre takeoff check of the airplane,
- No technical check of the airplane was made since last October 4,
- The total weight of the aircraft was 160 kg above MTOW.

The aircraft departed Sverdlovsk with three passengers on board, three crew members, a load of 257 kg of fret and 38 kg of mail. Few minutes after takeoff, the engine failed. The crew reduced his altitude and attempted an emergency landing when the aircraft crashed in an open field located in Aramil, southeast of Sverdlovsk Airport. All six occupants evacuated safely and the aircraft was damaged beyond repair.
Probable cause:
Engine failure caused by a malfunction of the crankshaft. It was determined that the total weight of the aircraft was 150 kg above MTOW, which caused the failure of the crankshaft because the engine was subject to high contribution.

The crew departed Yanaul at 1000LT on a schedule service to Sverdlovsk. En route, weather conditions worsened and the visibility was very low due to thick fog. Approaching the village of Ananino, the airplane passed over houses at a height of about 4 metres. Then the pilot initiated a turn when the airplane stalled and crashed in a field outside the village. The airplane was destroyed and the mechanic was killed. Seriously injured, the pilot was transferred to a local hospital but he died few hours later.
Probable cause:
It was determined that the accident was the consequence of a stall during a turn initiated at low height because the airplane' speed was too low. The Chief of the Yanaul Airport authorized the pilot to start the flight despite the weather conditions en route were considered as poor. Also, the weather bulletins transmitted to the crew were incorrects and conflicting.

The airplane departed Sverdlovsk Airport at 1440LT on a flight to Kazan with an intermediate stop in Yanaul, carrying two passengers and two crew members. While cruising at an altitude of 800 metres, weather conditions worsened with snow showers. The captain reduced his altitude to 300 metres when he encountered blizzard conditions. He eventually decided to attempt an emergency landing near the village of Kontuganovo. While circling at a height of about 50 metres, the left wing impacted a tall tree and the airplane crashed. All four occupants were injured, two seriously. Few hours later, the captain died from his injuries.
Probable cause:
It was determined that the weather bulletin transmitted to the crew prior to departure was not relevant and was reporting local rain showers only. The pilot attempted an emergency in a hilly terrain in very limited visibility.
