On a flight from Vancouver to the Zeballos Gold Mine, the single engine aircraft hit tall trees and crashed in a wooded remote area located near Alberni Canal, west of Port Alberni. Nine months later, on 9 March 1939, British Columbia police found the wreckage of the missing aircraft strewn at the foot of several tall trees in a bush-covered hill section near Alberni Canal (presently Alberni Inlet) about 13 miles (21 km) from Port Alberni. Inside the crumpled fuselage were the remains of the missing persons.

Shortly after liftoff, the aircraft rolled left and right due to strong winds. This caused the left wing to struck the icy ground and the aircraft to crash along the runway edge. There were no casualties.

En route from Vancouver to Bridge River, the pilot encountered poor visibility while flying at night. Too low, the airplane impacted the surface of Lake Anderson and crashed. Three passengers were killed and three other occupants were injured. The aircraft was destroyed.
Probable cause:
It was determined that the pilot departed Vancouver too late with only 20 minutes of daylight remaining while the flight to Bridge River should take one hour and 15 minutes. Any flights above mountainous area after dark were prohibited and the pilot failed to follow this procedure.
