While on a paratroopers mission, the floatplane landed on a lake located behind the enemy line then suffered a hole in the hull and sank. All eight occupants evacuated safely and the airplane was damaged beyond repair.

The airplane crashed for unknown reasons while taking off from an airstrip somewhere in Russia while on a special night mission. All three crew members were rescued.

The airplane departed Schippelbeil (now Sępopol, Poland), on a flight to Borisov, Belarus. It crashed in unknow circumstances somewhere in Russia. All four crew members were killed.

The airplane was lost without trace and probably crashed in the region of the North Front while on a supply mission.

The right engine failed in flight. The airplane lost height, impacted trees and crashed in a wooded area. The pilot was injured and the mechanic was unhurt. The airplane was destroyed.

The crew was performing a transpolar flight from Moscow to New York with intermediate stops in Fairbanks and Chicago. While flying probably over the north part of Siberia, maybe over the Arctic Ocean, the aircraft disappeared after all radio communications ceased. Important SAR operations were initiated but were eventually suspended few weeks later as no trace of the aircraft nor the six crew members was found. Among the six aviators was the famous Soviet aviation pioneer Sigismond Levanevsky.
