Circumstances:
The airplane was engaged in a cargo flight from Cochrane to Villa O'Higgins, carrying six tons of various goods. After takeoff from Cochrane Airport, while initial climb, the airplane rolled to the right, lost height and crashed on a small hill located 3 km from the airfield. The aircraft was destroyed and all five occupants were killed.
Probable cause:
Failure of the right engine after rotation for unknown reasons.

Upon landing at Cochrane Airport, the right tire burst. Both main landing gear collapsed and the airplane skidded on its belly and came to rest. All three occupants escaped uninjured while the aircraft was not repaired.

The right tire burst upon landing.

Circumstances:
The crew was completing a training sortie out from Cochrane. The aircraft lost height and commenced low flying then crashed into a cable stretched across the Bow River at a height of 25 to 50 feet. The machine struck the water and carried on to the river bank a distance of about a hundred yards. The craft was completely wrecked and both occupants were immediately killed.
