The airplane departed RAF Waddington in the evening of August 12 on an operation to Berlin. Approaching the target area, it was shot down by the German Flak and crashed in a field located in Grossbeeren, some 15 km south of Berlin. Five crew members were killed and a sixth was injured and became PoW.

The airplane departed RAF Waddington at 0015LT on an operation to Berlin. Passing over the Netherlands at night, it was shot down by a DO.215 and crashed in a field located in Slochteren, some 16 km east of Groningen. All six crew members were killed.

The crew was engaged in a training mission at RAF Grantham. While circling in the vicinity of the airbase, the airplane suffered an engine failure. The pilot ordered the wireless operator to bail out and attempted an emergency landing when the airplane crash landed in a field. The pilot was killed while the wireless operator was rescued.

The crew was involved in a test flight at RAF Waddington. Shortly after takeoff, while climbing, one of the engine failed. The crew lost control of the airplane that crashed just beyond the airfield. A crew member was killed while two others were seriously injured. The aircraft was wrecked. F/S Arnott died from injuries sustained on July 20.

The crew was performing a test flight from Hucknall to Ternhill on behalf of the Avro Aircraft Company. On final approach to RAF Ternhill, the aircraft stalled and crashed few hundred yards short of runway. Three crew were injured while the TP R. Kirlew was killed.

Circumstances:
The airplane departed RAF Waddington with a scratch crew to carry out an engine test, following recent grounding of the Manchesters. An engine fire developed while over the Bristol Channel, but by skilful airmanship, S/L McIntosh succeeded in a crash-landing at Perranporth Airfield, Cornwall. The airframe was later converted for instructional use. All eight occupants evacuated safely.
