Circumstances:
Shortly after takeoff from Wamena Airport, while in initial climb in limited visibility due to low clouds, the aircraft struck power cables and crashed, bursting into flames. A pilot was killed while both other occupants were killed.
Probable cause:
It was reported that the aircraft failed to gain sufficient altitude after liftoff as it was overloaded.

Circumstances:
During the takeoff roll, one of the engine failed. The pilot abandoned the takeoff procedure and initiated an emergency braking maneuver. Unable to stop within the remaining distance, the aircraft overran and came to rest. The pilot, sole on board, was uninjured while the aircraft was damaged beyond repair.
Probable cause:
Engine failure during takeoff for unknown reasons.

Circumstances:
While cruising in poor weather conditions, the airplane struck Mt Puncak Jaya (4,160 meters high) located in Papua. As the airplane failed to arrive at Biak Airport, SAR operations were conducted but eventually suspended few days later as no trace of the aircraft nor the 8 crew members was found. The wreckage was located in 1969 on the south slope of the mountain.
