Circumstances:
The pilot took off from Anvers (Belgium) at around 14 h 40 bound for Saanen (Switzerland) where he was supposed to drop off his three passengers. The flight was performed in IFR then VFR, at a cruise altitude of 26,000 ft. After about 1 h 15 min of flight, still under IFR, the Geneva controller cleared the pilot to descend towards FL 210 and to fly towards Saanen with a view to an approach. A short time later, the radar data showed that the aeroplane deviated from the planned trajectory. Following a question from the controller, the pilot said he had an autopilot problem. The controller then asked the pilot to follow heading 165°, which the pilot read back, then asked him ‘‘are you okay, okay for the safety, it’s good for you? ‘‘. The pilot answered that he had a ‘‘big problem’’. The radar data show tight turns on descent. During these manœuvres, in reply to a request from the controller, the pilot said that he was in ‘‘total IMC’’. During this communication, the aeroplane‘s overspeed warning could be heard. The aeroplane was then descending at 15,500 ft/min. About ten seconds later, it was climbing at 15,500 ft/min. The right wing broke off about twenty seconds later. The indicated airspeed was then 274 kt and the altitude was 12,750 ft. The wreckage was found in woods in the commune of Solemont (25). A piece of the right wing was found about 2.5 km from the main wreckage. Some debris, all from the right wing, was found on a south-west/north-east axis. The aeroplane part that was furthest away was found 10 km from the main site. The aircraft disintegrated on impact and all four occupants were killed.
Probable cause:
The in-flight failure of the right wing was due to exceeding the aeroplane’s structural limits (ultimate loads) during loss of control by the pilot. In the absence of any flight recorders, the investigation was not able to determine the causes of this loss of control. It is possible that it may have been induced by a loss of situational awareness by the pilot at the controls of an aeroplane affected by an icing phenomenon. This phenomenon may have affected the aeroplane’s wings or an area around the roll control.

Circumstances:
The twin engine aircraft was engaged in an ambulance flight from Besançon to Amiens with 2 surgeons, one pilot and one operator agent. Following a course of 950 metres on runway 23 at Besançon-La Vèze Airport by night, the aircraft lifted off. With a low climb gradient, the aircraft collided with trees and crashed in a wooded area located 250 metres past the runway end, bursting into flames. The aircraft was totally destroyed and all four occupants were killed. Both surgeons were en route to Amiens in order to a lever harvesting.
Probable cause:
The lack of flight recorders made it impossible to trace the chain of events on board the aircraft. As a result, the causes of the accident could not be determined with precision. However, at least two scenarios could simultaneously explain the length of the takeoff roll and the low height after rotation: a lack of control of the airplane by the pilot, either by poor adjustment of the elevator trim or because his attention would have been focused inside the cockpit by any event, without reaction from the pilot passenger seated on the right. This scenario is consistent with his relative inexperience with the type of aircraft. The second scenario could be based on an inappropriate decision to seek significant speed after take-off or improvised instruction, neither pilot being aware of the obstacle constraints of the airfield. The significant obscurity, the operating specificities in medical transport and the presence of a second pilot with a status and role not provided for in the operations manual, without there being therefore any crew or distribution of tasks within the company's crew, are likely contributing factors. The 48-year-old pilot had a total of 3,500 flight hours, including 450 on type. The company agent who was seating on the right was a professional pilot who did not have a license on this type of aircraft and took advantage of the flight, in agreement with the corporate management, to acquire experience in a view to his future qualification on this type of airplane. He had a total of more than 4,000 flight hours.

Circumstances:
During the takeoff roll from Besançon-La Vèze Airport runway 24, the captain realized that the aircraft was not accelerating properly and decided to abort. The crew initiated an emergency braking procedure but the aircraft was unable to stop within the remaining distance. It overran, stuck a 2,4 metres high embankment located 30 metres past the runway end and came to rest, bursting into flames. The passenger was seriously injured and both pilots were killed.
Probable cause:
The accident was the consequence of a late decision by the crew to abandon the takeoff procedure completed with the parking brake not fully released. The following contributing factors were reported:
- Imperfect application of the checklist, not allowing the crew identify the anomaly when starting up or rolling.
- Bad weather conditions, which led the crew to concentrate on maintaining the runway centerline to the detriment of other checks, and make it impossible to see sufficiently early the landmarks likely to attract his attention to the insufficient acceleration of the aircraft.
- The position of the "BRAKE" warning light outside the pilot's normal visual circuit while taking off.

Circumstances:
The airplane departed Montbard-Touillon Airfield at 2220LT on June 4 to bomb a BMW factory in Munich. While returning to his base at night, after being damaged by the German Flak, the captain attempted an emergency landing in an open field when the aircraft crashed and exploded. All four crew members were killed.

Circumstances:
After completing a reconnaissance training exercice at night, the crew was returning to Pontarlier Airport. Upon landing, the airplane suffered a landing gear failure and crashed. All four crew members evacuated safely and the aircraft was damaged beyond repair.
