Circumstances:
On approach to Rahachow, the airplane was mistakenly shot down by the pilot of a Soviet fighter and crashed in an open field, bursting into flames. The crew escaped uninjured and the airplane was destroyed by fire. At the time of the accident, the city of Rahachow was attacked by the German troops.
Probable cause:
Mistakenly shot down by a Soviet fighter.

On approach to Babruysk Airport following a ferry flight from Kazan, the airplane was mistakenly shot down by the pilot of a Soviet fighter and crashed. All three crew members were killed.

Circumstances:
The pilot was engaged in a local training flight at Minsk Airport. Following several circuits in the vicinity of the airfield, he initiated the approach. Not properly aligned with the runway, the pilot decided to make a go around. At a height of about 50 metres, the airplane continued for about 300 metres then lost height, rolled to the left and crashed. The pilot was seriously injured and died seven hours later.
Probable cause:
It was determined that the pilot failed to position the stabilizers in a correct position for landing (negative instead positive angle). He was not familiarized with the aircraft and his experience on type was insufficient with 13,8 hours of flight. It was also reported that the instructor released the pilot too early for a solo flight.

The crew was attempting a world tour from the US via Russia. While overflying Belarus, the crew encountered unknown technical problems and attempted an emergency landing. The airplane landed hard, causing a part from the cockpit panel to separate. Control was lost and the airplane crashed. Both occupants evacuated safely and the aircraft was damaged beyond repair. Some parts were used to built another Lockheed Vega with MSN 118.
