Circumstances:
En route from Yuzhno-Sakhalinsk to Khabarovsk, while cruising at the assigned altitude of 10,600 metres, about 35 minutes after takeoff, the aircraft rolled to the right, entered an uncontrolled descent and crashed in a mountainous and isolated area located in the Sikhot-Alin Mountain Range. SAR operations were initiated but suspended a day later due to the deterioration of the weather conditions and started again four days later. The wreckage was found 11 days later about 50 km west of Grossevichi. The aircraft disintegrated on impact and all 98 occupants were killed.
Probable cause:
It was determined that the crew selected the left main fuel tank to feed all three engines after takeoff from Yuzhno-Sakhalinsk. During 35 minutes of flight, the automatic pilot system was able to control and counteract the imbalance caused by the fuel consumption on one side only. Eventually, the aircraft started to bank right and the crew reaction to an unexpected situation was non compliant, causing the aircraft to enter an uncontrolled descent.

Circumstances:
The aircraft was completing a flight from Poliny Osipenko to Nikolayevsk-on-Amur with intermediate stops in Kherpuchi and Oktyabrsky. Nine minutes after his departure from Kherpuchi Airport, the crew encountered poor weather conditions with low clouds. While cruising at an altitude of 400 metres, the aircraft struck tree tops, stalled and crashed in a wooded area located 21,5 km from Kherpuchi, bursting into flames. A passenger was seriously injured while 13 other occupants were killed.
Probable cause:
It was determined that the accident was the consequence of a controlled flight into terrain. The following contributing factors were reported:
- The crew continued the flight at the altitude of 400 metres while the minimum safe altitude for the area was fixed at 800 metres,
- Poor visibility due to low clouds (600 to 1,500 metres),
- The crew failed to return to the departure point when weather conditions deteriorated,
- Loss of visual contact with the ground after the crew decided to continue under VFR in IMC conditions,
- The crew did not receive the emergency radio station and signal devices before departure, as well as the fact that on that day the dispatch microphone at the Kherpuchi Airport was not turned on, which made it impossible to establish the actual instructions of the flight director. In such conditions, the flight was not monitored.

Shortly after takeoff, while in initial climb, the crew lowered the gear when the aircraft stalled and struck the runway surface. It slid for few dozen metres and came to rest. All 14 occupants escaped uninjured while the aircraft was damaged beyond repair.

The aircraft departed Belkachi at 0851LT on a cargo a flight to Aiykam (Ayan district of the Khabarovsk region) with a load of diesel fuel on board for a total weight of 1,100 kilos (ETA in Aiykam was 0955LT). En route, weather conditions deteriorated and the crew continued the flight into clouds. After the crew deviated from the planned flight path, he missed Aiykan Airfield and elected to return. While cruising at an altitude of 1,090 meters in clouds, the aircraft struck the eastern slope of a mountain (1,100 meters high) located some 10 km east of Aiykam, at N58°00'15" E134°10'09". The aircraft was destroyed by impact forces and post crash fire and both occupants were killed.
Probable cause:
Controlled flight into terrain after the crew decided to fly in limited visibility conditions below the minimum prescribed altitude.

During the takeoff roll, the captain decided to abandon the procedure for unknown reasons and initiated an emergency braking maneuver. Unable to stop within the remaining distance, the aircraft overran, lost its undercarriage and came to rest 218 meters further. All 46 occupants escaped uninjured while the aircraft was written off.

The crew started the takeoff procedure from a snow covered runway with a tailwind component. The aircraft lifted off at the end of the runway then collided with two airport vehicles. It nosed down and crashed on a car workshop located near the airport, bursting into flames. All nine occupants were killed.
Probable cause:
It was determined that the accident was the consequence of a poor flight preparation on part of the crew who took wrong decisions as well as airport authorities. The following findings were reported:
- The runway was contaminated with wet snow and whose thickness exceeded the permissible limit,
- The crew decided to takeoff with a tailwind of 21 km/h, which made the runway distance insufficient given the flight conditions,
- The runway was not properly cleared of snow,
- The crew failed to abandon the takeoff procedure in a timely manner.

Circumstances:
The aircraft departed Khabarovsk on a mail flight to Kurung, a gold mine located in the Konder Mountain Range. While flying at a height of about 15 meters, the captain asked the passenger and the third pilot to jettison the mail bags when the aircraft struck a high larch and crashed, bursting into flames. Both pilots were killed while both other occupants were injured. The mission did not provide to jettison the mail bags but was arbitrarily changed by the captain.
Probable cause:
The crew misjudged the distance with trees while jettisoning mail bags at low height.
