Circumstances:
Shortly after takeoff from Greensboro, while engaged in a local demo flight, the twin engine airplane went out of control and crashed in flames. All six occupants were killed.
Probable cause:
Uncontrolled descent following a failure of the elevator tab control system. The following factors were reported:
- Flight control system disconnected,
- Bolt connecting aft elevator bellcrank and elevator pushrod came loose,
- 100 h ours inspection completed 66 hours prior to the accident.

Circumstances:
The pilot, sole on board, was involved in a local test flight. Shortly after takeoff from Kankakee-Greater Kankakee Airport, while climbing, both engines failed simultaneously. The pilot reduced his altitude and was able to complete an emergency belly landing in a field. The airplane slid for several yards and came to rest. The pilot was unhurt while the aircraft was damaged beyond repair.
Probable cause:
Failure of both engines due to a fuel starvation.
