Circumstances:
The single engine aircraft departed from a small airstrip in Bishkek on a charter service (flight GRA4571) to Bukhara with two passengers and two pilots on board. About a minute after takeoff, while in initial climb, the engine lost power. The crew informed ATC about the situation and was cleared for an immediate return. Shortly later, the engine failed and the crew attempted an emergency landing in an open field located near the aerodrome. Upon landing, the aircraft collided with a small drainage ditch, lost its left main gear and both left wings before coming to rest. All four occupants escaped with minor injuries and the aircraft was damaged beyond repair.
Probable cause:
The reason for the loss of engine power in flight, most likely, was a reduction in the filing of the gasoline due to clogging of fuel filters. The appearance, structure and nature of the impurities found in the fuel on the aircraft were not characteristic of impurities accumulated in the fuel supply system in normal operating conditions. The source of the impurities in aircraft fuel system could not be found.

Circumstances:
18 seconds after it started the takeoff roll, after a distance of 360 meters, while at a speed of 150 km/h, the copilot pointed out a problem with the nose gear and asked if the rudder and elevators were unlocked. At this time, the captain realized that a takeoff was not possible and instructed to abandon the takeoff procedure. Engine power was reduced and an emergency braking procedure was initiated. At this time, the aircraft's was 300 meters from the runway end at a speed of 215 km/h. 18 additional seconds elapsed from the copilot's remark. Unable to stop within the remaining distance, the airplane overran, rolled for 540 meters then collided with an embankment and came to rest in flames against trees. 12 occupants were injured while seven passengers were killed. The aircraft was partially destroyed by a post crash fire.
Probable cause:
It is believed that control column was blocked and a take-off was impossible in such conditions. Various technical investigations could not precisely determine the causes of this anomaly which could be consecutive either to a human error or to the malfunction of the system as a result of the presence of corrosion on connectors, which would affect the alarm of the locking system. The decision to abort the takeoff procedure was taken too late, which was considered as a contributing factor.

Thirty minutes after its departure from Bukhara Airport, the right engine stopped due to the failure of the cylinder n°2. The crew informed ATC and decided to continue to Tashkent on one engine at the altitude of 2,400 meters. The flight on one engine continued for one hour and 39 minutes when the power on the left engine was inadvertently reduced by the crew. At a height of 100 meters, the crew elected to restart the right engine but the propeller went into pitch position, causing high drag. The airplane lost height and crashed in a cotton field located 34 km south of Tashkent-Yuzhny Airport. 29 people were wounded while nine passengers were killed. A day later, two other passengers died from their injuries.
Probable cause:
Failure of the second cylinder on the right engine due to poor manufacturing. A series of omissions, errors and wrong decisions on part of the flying crew led to a difficult and uncontrollable situation.
