Circumstances:
The pilot, sole on board, was completing a cargo flight to Milroy, Pennsylvania. While on approach in poor weather conditions (low ceiling and rain), both engines failed simultaneously. Unable to maintain the prescribed altitude due to an insufficient speed, the pilot attempted an emergency landing in an open field when the aircraft crashed few miles from the airport. The aircraft was destroyed and the pilot was injured.
Probable cause:
Double engine failure on approach due to a fuel starvation caused by a mismanagement of fuel and inattentive to fuel supply on part of the pilot.

Circumstances:
While approaching Kahului Airport by night under VFR mode, the pilot got disoriented and was unable to locate Kahului Airport. He flew past the destination over dark unlighted terrain when the twin engine airplane struck a hill and crashed in flames. The pilot was killed. At the time of the accident, the ceiling was about 2,000 feet with rain showers.

Shortly after takeoff from Fort Lauderdale-Bradley Airport, while climbing,both engines stopped. The airplane stalled, dove into the ground and crashed in a huge explosion. Both occupants were killed.
Probable cause:
Failure of both engines after takeoff due to a fuel starvation, a mismanagement of fuel and an inadequate inspection of the aircraft prior to takeoff on part of the pilot. It was determined that the fuel selector valve decal was missing and the pilot made erroneous determination of the valve position.
