Circumstances:
While descending to Atlantic City-Pomona Airport, the crew completed a 350° turn to join the approach path when control was lost. The airplane stalled and crashed in flames in Egg Harbor, few miles from the airport. The aircraft was destroyed and both pilots were killed.
Probable cause:
Failed to maintain flying speed on initial approach. The following contributing factors were reported:
- Airframe ice,
- Icing conditions including sleet and freezing rain,
- Crashed in steep nose down attitude during a 350° turn heading change to intercept localizer.

Circumstances:
Shortly after takeoff from Middletown-Summit Airpark, while in initial climb, the twin engine aircraft banked left and crashed in a huge explosion. Both occupants were killed and the aircraft was destroyed.
Probable cause:
Lower cap of left wing failed approximately 8 inches outboard of the outer panel attach point, causing the left wing to detach. Fatigue fracture.

Circumstances:
En route from Pottstown to Middletown, while cruising at night, the crew encountered poor weather conditions with heavy rain falls. In limited visibility, the airplane impacted the slope of a mountain located 11 km northwest of Fredericksburg. The aircraft was destroyed and all five crew members were killed.
Probable cause:
Controlled flight into terrain.
