Circumstances:
The crew was engaged in a crop-spraying mission for the "Zauralny" sovkhoz located in the Kvarkeno district of the Orenburg region. The inexperienced pilots got distracted from piloting by listening to the weather forecast on the radio and writing it down. So the aircraft continually lost height and eventually crashed into a hill located one km southwest of Urtazym. The copilot was injured and the captain was killed.
Probable cause:
The crew was distracted and inexperienced. Also, he failed to prepare the flight according to the published procedures.

Circumstances:
The crew, consisting of one instructor and three pilots, was completing a local training flight in Buguruslan-Baymakovo Airport. A first training program was completed at an altitude of 1,000 meters and the instructor ordered the crew to return to the airport. On final approach, during the last segment, the instructor ordered the crew to go around and increased power on both engines. During the initial climb, while at a height of 150 meters, the instructor shut down the right engine and feathered its propeller. At the same time, the aircraft banked left up to 30°, stalled, dove into the ground and crashed in a huge explosion. The aircraft was destroyed by impact forces and a post crash fire and all four crew members were killed.
Probable cause:
It was determined that the aircraft stalled during initial climb due to the combination of flaps deployed at an angle of 17° and the power reduction on the right engine. Unfortunately, the distance between the aircraft and the ground was insufficient to a expect any recovery. The instructor decided to make a single engine climb out in violation of the NPP-47 training program and instructions published for IL-12. Wrong interpretation of the situation on part of the rest of the crew also contributed to the loss of control. The following factors were considered as contributory:
- Absence of appropriate measures concerning pilots management and training,
- Lack of control and demands in relation to the flight crews.

While flying from Astrakhan to Chkalovsk, the crew encountered poor weather conditions with snowfalls and decided to divert to the Sol-Iletsk Airport. On final, the aircraft crashed on a snowy hill located between Boyevaya Gora and Saratovka. All occupants survived the crash but unfortunately, nine of them froze to death within few hours.

Crash of a Tupolev ANT-40 near Sok-Karmala: 1 killed

Circumstances:
When weather conditions improved, the crew was cleared for takeoff and departed Kuybyshev Airport at 1127LT on a flight to Sverdlovsk. The pilot continued the flight at an altitude of 50 metres and once he arrived over the village of Sok-Karmala, he encountered foggy conditions. In low visibility, the airplane impacted trees, lost height and crashed 135 metres further, bursting into flames. The flight engineer was seriously injured and the pilot was killed.
Probable cause:
Failure of the pilot who continued the flight at an unsafe altitude in insufficient visibility caused by foggy conditions until the airplane collided with obstacles.

Circumstances:
The pilot, sole on board, departed Moscow on a postal flight to Tashkent with intermediate stops in Kuybyshev and Orenburg. On the leg to Orenburg, the pilot encountered poor weather conditions with thunderstorm activity and heavy rain falls. He decided to continue, descended to an altitude of about 400-500 metres when control was lost. The aircraft crashed in a wheat field located 4 km south of the village of Syrt, some 40 km northwest of Orenburg Airport. The aircraft was destroyed and the pilot was killed.
Probable cause:
Loss of control while flying in stormy weather and turbulences.
The following contributing factors were identified:
- Poor flight preparation as the pilot departed Kuybyshev Airport without any information about weather conditions at destination,
- Decision of the pilot to continue in unfavorable weather conditions rather than to return to Kuybyshev or to land asap.
