Circumstances:
On a cargo flight from Salmon to Indian Creek, the pilot made a detour to fly over a large game as the passenger was an avid hunter. While cruising at low altitude and low speed, the single engine airplane stalled, crashed and burst into flames. Both occupants were killed.
Probable cause:
Stall after the pilot failed to maintain flying speed. The following contributing factors were reported:
- Unwarranted low flying,
- High density altitude, about 9,000 feet,
- The crash site was not on the planned route.

Circumstances:
While flying at a relative low altitude into a blind canyon, the single engine airplane stalled and crashed between ridges on rising terrain. The wreckage was recovered in an isolated area two days later. All six occupants were killed.
Probable cause:
Stall and subsequent crash due to improper in-flight decisions on part of the pilot. The following contributing factors were reported:
- The pilot misjudged distance, speed and altitude,
- Flew into blind canyon,
- Unqualified person operated aircraft,
- Density altitude approximately 7,000 feet.

While approaching South Naknek, the pilot encountered poor weather conditions with low ceiling, fog and icing conditions. On final, the airplane struck the ground and crashed few miles from the airfield. The aircraft was destroyed and the pilot, sole on board, was seriously injured.
Probable cause:
Collision with dirt bank on approach after the pilot initiated flight in adverse weather conditions. The following contributing factors were reported:
- Ice on windshield,
- The pilot suffered a spatial disorientation,
- The pilot delayed in initiating a go-around,
- Low ceiling,
- Fog,
- Icing conditions including sleet, freezing rain,
- Downwind,
- Visibility two miles or less,
- Weather conditions below minimums.

While approaching Santa Barbara, the pilot encountered marginal weather conditions with low ceiling. The single engine airplane went out of control and crashed in the Pacific Ocean about 13 miles off Santa Barbara. SAR operations were initiated but the wreckage was never found.
Probable cause:
Uncontrolled collision with water after the pilot attempted operation with known deficiencies in equipment and suffered a spatial disorientation. The following contributing factors were reported:
- Failure or delay in initiating emergency procedures,
- Flight and navigation instruments: attitude gyro - erratic,
- Low ceiling,
- Visibility 4 miles or less,
- Artificial horizon malfunction prior to departure,
- Sprayed with wd-40,
- Adzd ATC enroute no gyro.

The single engine airplane departed Anchorage-Merrill Field on a taxi flight to Tyonek, on the Cook Inlet. Just after liftoff, while in initial climb, the engine lost power. The airplane lost height then struck a hangar and crashed. The aircraft was destroyed and all four occupants were killed. There were no casualties on ground.
Probable cause:
Engine failure during initial climb and subsequent collision with building due to poor maintenance servicing. The following contributing factors were reported:
- Improperly services aircraft by ground crew,
- Inadequate preflight preparation,
- Water in fuel,
- Partial loss of engine power,
- Water found in storage tank,
- Leak in fill pipe,
- Lineboy serviced aircraft while the storage tank being filled.

The single engine airplane departed Juneau Airport on a taxi flight to Hoonah, carrying two passengers and one pilot. While climbing in poor weather conditions, the pilot suffered spatial disorientation and lost control of the airplane that crashed in flames in an open field. The aircraft was totally destroyed and all three occupants were killed. At the time of the accident, weather conditions were poor with thunderstorm activity, low ceiling, rain and turbulences.
Probable cause:
Uncontrolled descent and subsequent collision with ground after the pilot continued VFR flight into adverse weather conditions. The following contributing factors were reported:
- Spatial disorientation,

Circumstances:
En route from Anchorage to Galena, the pilot encountered poor weather conditions with low ceiling. Lost and disoriented, he flew into a blind canyon when he saw high obstructions. While initiating an evasive maneuver to avoid collision, he made a steep turn to the left when the aircraft struck the ground and crashed in flames in a mountainous terrain located about 125 miles northwest from the city of Anchorage. As the airplane failed to arrive at destination, SAR operations were initiated but eventually abandoned few days later as no trace of the aircraft nor the pilot was found. On September 2, 1980, the burnt wreckage was found near Rainy Pass.
Probable cause:
Controlled flight into terrain after the pilot became lost and disoriented. The following contributing factors were reported:
- The pilot continued VFR flight into adverse weather conditions,
- The pilot misjudged distance, speed, altitude or clearance,
- Low ceiling,
- High obstructions,
- The pilot flew into blind canyon,
- Evasive maneuver to avoid collision,
- In-flight steep left turn,
- Recovery date September 2, 1980.
