The pilot was engaged in a local solo training flight at RAF Upwood. Just after takeoff, the airplane suffered an engine failure and crashed near the bomb dump. The pilot was uninjured while the airplane was damaged beyond repair.

The crew was completing a cross-country training exercise when the Wellington collided with a Hurricane registered V7225 of 1401 Meteorology Flight. The bomber dove into the ground and crashed in Prickwillow, killing the entire crew.

The starboard engine failed while the airplane was lifting off the ground. The crew continued to climb when, at a height of 100 feet, the port outer engine failed as well. The airplane descended to the ground and crashed one mile from the airbase. All crew escaped uninjured.
