Circumstances:
The pilot, sole on board, was completing a drug smuggling light and the aircraft was carrying a load of marijuana. Following a complete failure of both engines, the pilot attempted an emergency landing in a desert area when the airplane crash landed in a dry lake bed and hit a ditch. The aircraft was abandoned and the pilot was not found.
Probable cause:
Failure of both engines following a fuel exhaustion. The following contributing factors were reported:
- Mismanagement of fuel,
- Fuel exhaustion,
- Rough/uneven terrain,
- Illegal flight.

Circumstances:
Just after takeoff from Palma Sola, an engine failed. The pilot lost control of the airplane that collided with a fence. The pilot was uninjured and the aircraft was damaged beyond repair.
Probable cause:
The pilot attempted operation beyond experience/ability level. The following contributing factors were reported:
- Misjudged distance, speed, altitude or clearance,
- Spontaneous, improper action,
- Powerplant failure for undetermined reason,
- Lack of familiarity with aircraft,
- Taxiing on runway, unable to stop, attempted takeoff,
- Not multi engine rated, all times as copilot.

Shortly after takeoff from Dallas-Love Field Airport, while climbing, the pilot informed ATC about serious problems and elected to return for an emergency landing. He was cleared to do so and completed a turn to rejoin the airport when he lost control of the airplane that crashed onto several houses by the airfield. The aircraft was destroyed and the pilot was killed. There were no injuries on the ground.
Probable cause:
Stall and spin during traffic pattern circling after the pilot failed to maintain flying speed. The following factors were reported:
- Inadequate preflight preparation,
- Improperly loaded aircraft and CofG,
- Suspected mechanical discrepancy,
- CofG 8.9 inches after the rear limit.

Approaching by night and without any terrain in sight, the pilot lost control of the airplane that stalled and crashed few miles from the airport. The aircraft was destroyed and the pilot, sole on board, was injured.

Double engine failure in flight caused by fuel exhaustion. The following factors were reported:

Probable cause:
Double engine failure during initial climb following a mismanagement of fuel on part of the pilot. The following factors were reported:
- Fuel starvation,
- Failed to maintain flying speed,
- Inadequate preflight preparation,
- Failed to use the pre-departure checklist,
- Improperly loaded aircraft,
- Seats improperly installed,
- Takeoff with rear auxiliary tanks that were not serviced,
- Fuel selector valve configuration different than other aircraft flown,
- All cabin seats separated.

While cruising along the west coast of Florida on a cargo flight from Kingston to Tampa, the pilot informed ATC about an engine fire and elected to divert to Sarasota-Bradenton Airport for an emergency landing. On approach, the pilot realized he could not make it so he landed the airplane in a prairie. The aircraft rolled for few dozen yards before coming to rest in flames. While the pilot escaped uninjured, the aircraft was destroyed by fire.
Probable cause:
Engine fire in flight and precautionary landing off airport. Fire after impact. The pilot executed a wheels down landing. The cause of fire described as whitish in colour could not be determined.
