Circumstances:
Following an improper level off, the twin engine airplane landed hard, bounced several times and crash landed after the undercarriage collapsed. The pilot, sole on board, was unhurt.
Probable cause:
Gear collapsed upon landing following a hard landing due to improper level off and improper recovery from bounced landing. Overload failure was a contributing factor.

Circumstances:
On approach to Saint Louis-Downtown Airport, the crew encountered poor visibility with low ceiling and icing conditions. Too low, the twin engine airplane struck power cables and crashed in Centreville, near the airport. Three passengers were killed while three other occupants were injured.
Probable cause:
Collision with wires/poles on final approach due to improper IFR operation. The following contributing factors were reported:
- Low ceiling,
- Icing conditions including sleet, freezing rain,
- Visibility three miles or less,
- Fog,
- Hit wires during localizer approach.

Circumstances:
While cruising at an altitude of 21,000 feet in marginal weather conditions, the airplane went out of control, spiraled to the ground and crashed in a field. The aircraft was totally destroyed and both occupants were killed.
Probable cause:
Airframe failure in flight and uncontrolled descent for undetermined reasons. The following findings were reported:
- Overload failure,
- Separation in flight,
- Rudder, horizontal stabilizers, elevators, outboard section of left wing separated in flight.

Circumstances:
For unknown reasons, the crew selected the wrong runway to land at Wheeling-Palwaukee Airport (Chicago-Executive) taking into consideration the wind component. After touchdown, the captain elected to use all available brake systems when the copilot stowed reversers. Unable to stop within the remaining distance, the airplane overran, went through a fence and a road and eventually crashed in a warehouse. Both pilots as well as three people on the ground were injured. The aircraft was destroyed as well as two cars.
Probable cause:
Overshoot on landing after the crew misjudged distance and speed. The following contributing factors were reported:
- Improper operation of powerplant and powerplant controls,
- Spontaneous improper action on part of the copilot,
- Poor crew coordination,
- Selected wrong runway relative to existing wind,
- Powerplant - thrust reverser,
- Material failure,
- Asymmetrical thrust reversal,
- Right engine flex shafts failed.

After takeoff from Chicago-Merrill C. Meigs Airport, while climbing, the airplane collided with a flock of seagulls. An engine lost power and the airplane lost speed and height. The pilot elected to return when the airplane stalled and crashed into Lake Michigan. The aircraft was destroyed and all four occupants were killed.
Probable cause:
Stall and spin after the airplane collided with a flock of seagulls. The following contributing factors were reported:
- Exercised poor judgment,
- Bird ingestion,
- Failed to maintain flying speed,
- Airport supervisory personnel,
- Improper emergency procedures,
- Pilot advised of seagulls on runway,
- Ineffective bird removal technique,
- Failed to retract flaps.
