Circumstances:
On approach to Oklahoma City-Will Rogers Airport, the twin engine airplane stalled and crashed in flames in Newcastle, about 8 miles south of the airport. The aircraft was destroyed and both pilots were killed.
Probable cause:
Stall on approach after the crew failed to maintain flying speed. The following contributing factors were reported:
- Inadequate/incorrect weather briefing,
- Icing conditions including sleet, freezing rain,
- Airframe ice,
- Forecast of icing conditions not provided to pilot.

Circumstances:
En route from Newcastle to Liverpool, the pilot was facing clouds and initiated a sharp turn when he lost control of the airplane which suffered a structural failure, entered a dive and crashed in a field. All three occupants were killed.
Probable cause:
Structural failure of the rudder following a sharp turn attempted by the pilot.

Circumstances:
While cruising over Northumberland, weather conditions deteriorated. The pilot elected to return to Newcastle Airport but was eventually forced to attempt an emergency landing in an open field. The airplane crash landed in boggy ground, collided with a water tank and came to rest. The pilot evacuated safely and the aircraft was damaged beyond repair.
