The crew was completing a patrol mission when he encountered poor visibility. While cruising in fog, the floatplane impacted the slope of Mt Elkhorn located 25 km northwest of Staunton. All 7 crew members were killed.
Probable cause:
Controlled flight into terrain in foggy conditions.

While on a maritime patrol flight, the seaplane was shot down by a Japanese Nakajima Ki-27. The crew reduced his altitude in ditched the airplane off Sumatra. The airplane sank and all occupants survived.

The floatplane was engaged in an operation over the Bali Island. While approaching Denpasar Airport at an altitude of 150 feet, it was shot down by a Japanese fighter and crashed, bursting into flames. All seven crew members were killed.

En route from Indonesia to Australia, the floatplane was attacked by the pilot of a Japanese Mitsubishi Zero. Bullets impacted the fuselage, fuel tanks and the right wing that caught fire. The captain reduced his altitude and attempted to ditch the airplane off Bathurst Island, north of Darwin. Upon touchdown, the seaplane overturned and sank quickly. Nevertheless, all 18 occupants were able to evacuate the cabin and were later rescued by the crew of the Philippines ship named 'Don Isidro' then disembarked on Bathurst Island.

After liftoff in poor weather conditions (a storm was passing over the area), the airplane clipped trees and crashed. A crew member was seriously injured and seven others were killed. Few hours later, the only survivor died from injuries sustained.
