The airplane departed RAF Scampton on an operation to Gelsenkirchen. Due to severe icing conditions, the captain decided to return. On approach to RAF Hemswell, the aircraft went out of control and crashed. The pilot was killed and three other crew members were injured.

Circumstances:
The airplane departed RAF Scampton in the evening of September 27 on an operation to Lorient. While returning at night, the crew got lost and were unable to locate RAF Scampton. Over Lincolnshire, the airplane was abandoned and crashed on the St Matthias Church in Lincoln. A crew was killed and three others were injured.

Shortly after a night takeoff from RAF Hemswell, the airplane collided with a second RAF Hampden registered P4397 and crashed. A crew was killed and three others were injured. The aircraft was destroyed. P4397 was repaired and later served with 14 OTU.
