Circumstances:
A Purdue Aeronautics Corporation, Douglas DC-3, N386T crashed while making an instrument approach to the Morgantown Municipal Airport, Morgantown, West Virginia. The accident occurred 2.5 nautical miles from the Morgantown VOR on the 340 degrees radial at 1110 c.s.t. 1 November 29, 1963. The stewardess was fatally injured in the accident but the captain and first officer escaped with minor injuries. There were no other persons aboard the aircraft. The aircraft was damaged beyond repair by the impact and post impact fire. N386T was being ferried to Morgantown in preparation for a charter flight.
Probable cause:
The Board determines that the probable cause of this accident was the pilot's execution of an instrument approach in an aircraft not equipped with navigational instrumentation appropriate to the ground facilities being used.

Circumstances:
The crew departed Southend in the UK on a delivery flight to Nigeria with intermediate stops at Marrakech, Agadir, Fuerteventura, Las Palmas and Dakar. While cruising by night, the crew decided not to land at Agadir and continued directly to Fuerteventura. While approaching the island, the crew elected to contact ATC but without success. Also, as the Fuerteventura NDB was OFF, the crew was unable to receive any signal and in such conditions, decided to return to Agadir. While approaching the Moroccan coast, the pilot was forced to ditch the aircraft 22 km off shore due to a fuel exhaustion. Quickly on site, the crew of a trawler was able to evacuate all three crew members while the aircraft sank and was lost.
Probable cause:
It was determined that the Fuerteventura NDB has been switched off for the night as the airport was closed to traffic. The crew filed the flight plan to Las Palmas before his departure from MAK but Fuerteventura Airport was already closed at that time. Thus it is believed the crew failed to prepare the flight according to procedures and failed to land at Agadir Airport as scheduled.

On a delivery flight from Prestwick to RAF Boscombe Down, both engines failed. The pilot elected to restart the engine but without success and eventually decided to attempt an emergency landing in a field located in Kinlet, Shropshire. While descending, the airplane struck trees and crashed. The pilot was uninjured and the airplane was damaged beyond repair.
Probable cause:
It was determined that both engines stopped because the fuel selector was connected to an empty tank while the starboard tank was full. Also, the approach configuration adopted by the pilot was improper, causing the airplane to struck trees.

The brand new twin engine airplane was on a delivery flight to the Government of Ceylon from Kansas City to Colombo with several intermediate stops. While approaching Naples-Capodichino Airport, the aircraft struck a hill located at Camaldoli, about 7 km short of runway 06 threshold. The airplane was destroyed and all three occupants were killed, the pilot William Palmer, his wife and one technician. For undetermined reason, the pilot was completing the approach at an insufficient altitude.

Circumstances:
After being manufactured at Frunze, the aircraft was on a delivery flight from Moscow to Irkutsk with intermediate stops at Sverdlovsk and Novosibirsk. On final approach to Irkutsk-Intl Airport, the crew encountered marginal weather conditions when the airplane hit the ground 1,315 meters short of runway, bounced, struck a fence and eventually came to rest in a prairie located 550 meters short of runway threshold. The copilot was killed while four other occupants were slightly injured. The aircraft was considered as damaged beyond repair.
Probable cause:
The flight engineer misinterpreted some instruments settings he failed to transmit to the flying crew in due time. Lack of coordination between pilots was considered as a contributing factor.
