Circumstances:
On final approach to Kaliningrad, cockpit crew encountered problems with flaps asymmetry and gear malfunction. A go around was performed and on second approach, gear was not selected down. Aircraft landed on its belly and skidded for 1,440 meters before coming to rest on runway. All 144 occupants were evacuated while the aircraft was damaged beyond repair.
Probable cause:
MAK concluded that accident was caused by several errors made by crew who forgot to deploy the landing gear during the second attempt to land.
It appears that following factors were considered as contributory:

-copilot turn off several circuit breaker on approach (GPWS and gear)
-crew failed to comply with landing checklist
-poor crew coordination and communication
-lack of crew resources management
-poor training
-emotional stress on part of the crew during the go around procedure.

Following this accident, both pilots will lose their licence and KD Avia's AOC will be cancelled by Russian authorities in September 2009. The same month, KD Avia bankrupted.
