Circumstances:
On a flight from Bykovo to Ulyanovsk when the crew forgot to extend the flaps before take-off and the landing gear was retracted too early so that the tail struck the runway. The captain tried to take off anyway but the aircraft was not able to lift off, hit the perimeter wall of the airport, crossed a road, collided with buildings and caught fire. All 5 crew and 6 out of 42 passengers injured.
Probable cause:
Wrong take off configuration and poor coordination on part of the crew.
