Circumstances:
On the leg from Pushkin to Ukhta of a flight from Pushkin to Norilsk when engine No. 4 failed during the take-off run (due to faulty maintenance). Crew did not realize this and continued the take-off. Aircraft started to turn and bank to the right while still being on the runway, took off at 238 km/h (with the recommended speed being 250 km/h) in order to prevent veering off the runway, the right wing struck the ground 2 seconds after lift-off. Aircraft fell back to the ground off the runway, broke up, caught fire and burnt out. 3 out of 7 crew injured while the other 4 and both passengers escaped unhurt.
Probable cause:
Engine failure due to poor maintenance.
